      What to Expect (Job Responsibilities):
      - Provide care coordination, case management, and care management services to members
      - Implement disease management programs, educating and empowering members to manage their conditions
      - Conduct triage and referral, transition of care planning, and end-of-life care planning for members with acute and complex needs
      - Assist members with managing health plan benefits and finding alternative services when needed
      - Work within the health plan framework to improve access and quality of care

      What is Required (Qualifications):
      - Minimum of 5 years of clinical nursing experience
      - Experience collaborating with physicians in managing patient care
      - Current unencumbered Registered Nurse License in the state of residency

      How to Stand Out (Preferred Qualifications):
      - Bachelor's Degree in Nursing or Health Education
      - Current unencumbered Registered Nurse License in Oregon upon hire
      - Current unencumbered Registered Nurse License in Washington State within six months of hire date
      - Experience in case management and utilization management
      - Familiarity with health plans, medical insurance plans, or similar environments
