What to Expect (Job Responsibilities):
- Provide care coordination and case management services to members of Providence Health Plans
- Educate, motivate, and empower members to manage their diseases effectively
- Collaborate with physicians to manage patient care and improve access to quality care
- Assist members with managing day-to-day functional needs and health plan benefits
- Work within the health plan framework to manage medical expenses and enhance care quality

What is Required (Qualifications):
- 5+ years of clinical nursing experience
- Current unencumbered Registered Nurse License in the State of Oregon
- Experience with electronic medical records
- Bachelor's Degree in Nursing or Health Education (Preferred)
- Case Management Experience (EX. Triage and Referral, Transition of Care Planning, Discharge Planning, Coordination of Outpatient Care) (Preferred)

How to Stand Out (Preferred Qualifications):
- Experience working at a health plan or similar environment
- Experience working with Medicaid and Medicare populations
- Current nursing experience in specific areas like cardiology, pediatrics, oncology, etc.
