      What to Expect (Job Responsibilities):
      - Perform timely pre-visit reviews of ambulatory encounters to identify and recapture medical conditions as HCC diagnoses
      - Accurately interpret clinical information in medical records to identify potential diagnoses
      - Educate providers on diagnostic specificity to enhance patient condition representation
      - Identify educational themes and process improvement opportunities through chart reviews
      - Maintain professional competency through ongoing education and training in coding and clinical documentation

      What is Required (Qualifications):
      - Minimum three (3) to five (5) years of experience in case management, outpatient coding, utilization review, or CDI
      - Experience in Primary Care, medical coding, risk adjustment, or CDI preferred
      - Current certification in Clinical Documentation Improvement (e.g., CDIP, CCDS) is preferred
      - Certification in medical coding or risk adjustment (e.g., CRC, CPC) preferred
      - Strong PC skills, including proficiency in Microsoft applications like Outlook and Excel

      How to Stand Out (Preferred Qualifications):
      - Medical licensure (RN, PA, NP) preferred
      - Bachelor’s degree in a healthcare-related field preferred
      - 2 years of experience specifically in Primary Care or CDI
