      Job summary

      The Clinical Site Manager provides Operational & Clinical Leadership within Derriford Hospital and manages the flow of patients on a day to day basis. To be able to liaise with all specialties and directorates concerning tertiary referrals, targets and waiting list initiatives, as well as maintaining patient flow across the Trust and the Emergency Department.

      The Clinical Site Manager will assist in running The Command Centre, which is a fast paced and exciting hub of activity. The Command Centre are a well integrated and motivated team who thrive on problem solving and creating solutions.       Main duties of the job

      The Clinical Site Manager will be responsible for the capacity overview and patient flow, ensuring the safe transfer of patients throughout the Trust and to external agencies. Ensuring that effective utilisation of the beds across the Trust, ensuring the elective and emergency admissions are accommodated in the most appropriate areas, ensuring the Trust adhere to the 4 hour target.

      The post holder will liaise with the front line directorates when capacity within the trust and patient flow is compromised. The Clinical Site Manager will be expected to form part of the response team in a major incident and take responsibility for this role in conjunction with other Senior Managers and Clinical Staff.

      ***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

      PRIMARY DUTIES & AREAS OF RESPONSIBILITY

      • Is responsible for the management and co-ordination of movements of patients within the Trust to any specialty in order to keep patient flow at its optimum and in order to meet government standards, targets and waiting times.
      • Responsible for running the patient flow meetings on weekdays and ensuring attendance is kept with the key stakeholders. Maintaining an action log from the flow meetings and ensuring actions are completed.
      • Responsible for producing accurate up to date bed states and maintain the accuracy for the visual hospital boards also known as Salus
      • Responsible for site management 7 days a week.
      • Responsible for managing the bed management team on a day to day basis, ensuring the site is managed appropriately throughout the day.
      • Responsible for challenging wards/ clinical teams when patients extend beyond their predicted date of discharge.
      • Responsible for escalating issues with capacity and demand with the on-call ED and on-call Manager, in line with the escalation process.
      • Contributes to the management of risk throughout the hospital in order for the Trust to maintain financially viable and to meet its targets.
      • Responsible for developing Site Management service with the Matron, which meets the needs of the Trust.
      • Responsible for the recruitment of staff within the team.
      • Conduct appraisals and professional development plans for the bed management team.
      • Liaise with Consultants and all the clinical staff to determine appropriate allocation of patients to beds.
      • Assess the level of care required by individual patients to ensure that they are treated in the right place by the right people.
      • Liaise with the critical care unit staff to repatriate patients onto ward beds as appropriate to their care needs.
      • Liaise with other hospitals to ensure the safe transfer of patients to and from the Trust.
      • Maintain accurate information regarding patient information paying particular attention to the patient discharge requirements.
      • Work with the delayed discharge team to ensure the site team are aware of delays and informed on when patients are being discharged.
      • Be responsible for the bed managers providing accurate and up to date information on the daily situation throughout the Trust regarding the bed state and patient flow.
      • Ensure a firm understanding of the Trusts waiting times, including the Inpatient and Emergency Department waiting times and ensuring the patients are moved onto appropriate wards..
      • Have a good working knowledge of databases used, the ability to analyse data as required for site management purposes.
      • Ensure a clear understanding is obtained surrounding Data Quality, Data protection, and the Patient information Systems (iPM) and HAS and SALUS.
      • Liaise with the On-call manager and Senior Nurse on a daily basis.
      • Responsible for the reporting of untoward incidents, complaints and clinical emergencies as detailed in Trust-wide protocols and procedures.
      • Leading own and contributing towards projects being run by the Trust in dealing with the patients flow through the Trust and efficiencies.
      • Ensuring that resources are used efficiently and in accordance with agreed procedures.
      • Undertake other duties within the post holders competences/career development plan as may be necessary from time to time, as determined by the matron.

      COMMUNICATIONS & WORKING RELATIONSHIPS

      • Ensure effective communication is maintained with clinical and non-clinical teams, thus developing professionalism through the Trust.
      • Responsible for the working relationships with the bed management team and the ward managers,
      • Ensure the information given communicated at flow meetings and to the Trust is accurate and up to date.
      • Responsible for ensuring the bed management team and discharge team work collaboratively in achieving the patient flow required to deal with the patients requiring beds on a day to day basis.
      • Liaise with other healthcare professionals and medical staff to ensure efficient and effective patient care. This may involve the use of complex information when discussing the patients condition to achieve the correct outcome.
      • Cover colleagues absence during periods of annual leave and absence.
      • Ensure bed managers from other sites and Trusts provide the relevant information on patient transfers and the Trust, when repatriating patients provide relevant information and transfer the patient safely.
