      Position Summary:

      The individual will provide clinical support to DBHIDS Homeless Services Unit including but not exclusively management of the safe haven referrals, outreach operations and clinical support of cases within the hospital system. This job will include other tasks as directed by the Homeless Services Unit Manager and/ or Director.

      Duties and Responsibilities:

      This individual will:

      • Lead the Safe haven system daily openings and referral process
      • Assist OCC Manager around Quality Assurance needs of teams, updating policy and procedure and agency staff listing
      • Assist OCC Manager and staff on summer and winter training
      • Provide and coordinate training, such as safe haven referral process and crisis intervention linkage
      • Provide follow-up with CRC and hospital from 302 performed but DBHIDS homeless teams. Working with hospital on proper discharge planning
      • Facilitate clinical case conference cross systems and or departments for some of the most vulnerable on the streets
      • Facilitate monthly focus list meetings and provide 1:1 clinical support to ensure teams are working on appropriate housing plans
      • After hour on-call responsibilities to support outreach services
      • Complete outreach with all DBHIDS funded teams to provide training and guidance. Provide a written summary of strengths and areas of improvement for team supervisors. This would rotate with all teams and shifts as required.
      • Assist with city initiatives that are requested of DBHIDS homeless unit
      • Support the Navigation center around programming needs
      • Support all overnight street counts throughout the year (4 per year)

      Skills Required:

      • Experience working with homeless adults and or / families
      • Strong working knowledge of mental health and substance use disabilities
      • Knowledge of multiple support and social service systems including the Office of Supported Housing and the Behavioral Health System
      • Excellent communication skills
      • Strong interpersonal skills
      • Excellent computer skills
      • Very strong organization and analytic skills; detailed oriented
      • Excellent computer skills including use of Microsoft Word, Excel, PowerPoint and Outlook

      Education and Experience:

      Bachelor's degree in Social Work, Human Services/ Social Services, Healthcare or related field required or 7 years experience with supportive housing services systems is a plus
