      Job summary

      South Tyneside Recovery at Home Team, based at Clarendon, Hebburn, has an exciting opportunity to employ Clinical Team Manager. The team is MDT including advanced care practitioners, nurse practitioners, nurses, occupational therapists, physiotherapists, health care assistants and administrative staff. The team covers urgent and planned care in South Tyneside Community over 7 days 24 hours a day. This post demands someone with strong transformational leadership skills, drive and enthusiasm, organised and a willingness to develop themselves and the team. You need a professional qualification in health with at least 3 years post qualification experience in practice relevant to your qualification. Evidence of leadership, change and management skills are also essential.The successful candidate must possess excellent communication and people skills, a non-judgmental and person-centered approach and above all a real caring attitude and be prepared to make a difference. Applicants should hold a current 1st level registration in nursing or a registered physiotherapist or occupational Therapist, along with a health-related degree. This post is substantive.       Main duties of the job

      To be accountable for the overall management and development of the team / service, deployment and supervision of staff and ensuring the coordination of services provided, optimising the cost effective use of all resources. Line management of the team, with overall accountability and responsibility for the delivery of all aspects of patient / client care through the provision of effective clinical and managerial leadership. To lead, motivate and support the team approach to delivering new and innovative models of care for patients / clients. To lead a continuously improving service, by ensuring systems are in place to support clinical governance, quality monitoring and ongoing research and audit of practice. Ensure systems and processes are in place to maintain patient safety. Facilitate the development of clinical expertise and advanced clinical practice within the team. To work autonomously, assessing individual patient / client needs, initiating investigations, determining a plan of care and initiating appropriate holistic, research based health information and care. To act as a resource for all members of the Multi-Disciplinary Team, providing expert advice concerning aspects of patient / client management. To be responsible for ensuring that agreed professional standards are reflected in practice and to have overall responsibility for monitoring the standard of care.
