      Responsibilities

      What You Will Do:

      • Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
      • Complete patient assessments of need

      • Support homecare operations within the community

      • Act as the face of the organization while in the community

      • Provide care insights to patients and caregivers while in the field

      • Timely complete records in accordance with Company policy

      Qualifications

      • High school diploma
      • Valid drivers’ license
      • CNA state certification (required)
      • Prior experience as a CNA (required)
