- Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
Complete patient assessments of need
Support homecare operations within the community
Act as the face of the organization while in the community
Provide care insights to patients and caregivers while in the field
- Timely complete records in accordance with Company policy
- High school diploma
- Valid drivers’ license
- CNA state certification (required)
- Prior experience as a CNA (required)
