      Main duties of the job

      As a key member of our Home Visiting Team, you'll play a vital role in shaping and delivering outstanding care across North East Derbyshire. Your main responsibilities will include:

      • Providing clinical leadership and supervision to support and guide the Home Visiting Team.
      • Carrying out home visits for complex cases, including end-of-life care and advanced prescribing needs.
      • Leading senior clinical triage for the Acute Home Visiting service, ensuring timely and effective responses.
      • Collaborating with the Local Navigation Hub and contributing to multidisciplinary team meetings to ensure joined-up care.
      • Delivering holistic assessments and advanced care planning, helping to reduce avoidable hospital admissions and improve patient outcomes.
      • Please note: there may be a need to work a longer day to support the Local Navigation Hubs details to be confirmed.

      • Demonstrable interest and experience in elderly care
      • Understanding of adult safeguarding and Deprivation of Liberty Safeguards (DoLS)
      • Experience in multidisciplinary team working and senior clinical triage

      Desirable

      • Experience with Comprehensive Geriatric Assessment (CGA)
      • Experience in clinical supervision or mentoring
      • Familiarity with SystmOne clinical system

      Qualifications

      • GMC registration with a licence to practise
