- Providing clinical leadership and supervision to support and guide the Home Visiting Team.
- Carrying out home visits for complex cases, including end-of-life care and advanced prescribing needs.
- Leading senior clinical triage for the Acute Home Visiting service, ensuring timely and effective responses.
- Collaborating with the Local Navigation Hub and contributing to multidisciplinary team meetings to ensure joined-up care.
- Delivering holistic assessments and advanced care planning, helping to reduce avoidable hospital admissions and improve patient outcomes.
