      Responsibilities

      The primary purpose of the position is to provide one on one assistance to persons with special needs. We provide specifically tailored life plans/goals to enhance the developmental skills that is needed for those to live independently within their chosen community.

      • The Community Habilitation Specialist displays a congenial, warm acceptance for all individuals.
      • The Community Habilitation Specialist will primarily be responsible for the assessment andobservation of the person’s skills development. Is alert to the expressions of attitudes, feelings and relationships of each individual. Develops an awareness of the individual’s needs, not only through the observation and interaction but through a study of the individual’s case history and by appropriate consultation with other members of the staff.
      • Support the person in navigating the program and identifying incentives in learning new skills andbehaviors.
      • Engage primary caregivers and other key participants in active change-oriented treatment byidentifying and overcoming barriers to engagement.
      • Implement problem conceptualization, treatment planning, intervention implementation, andoutcome review and strategy revision procedure using the model's Analytic process (Brief FamilySystems Therapy and Parental Management Training).
      • Develop Valued Outcomes with the steps on how to achieve it based on caregivers/communitysupports and behaviors observed along with child/adolescent strengths.
      • Community Habilitation Specialist meet with the assigned children/adolescent for 1-2 times perweek depending on the child/adolescent coordinated hours
      • Community Habilitation Specialist will travel to borough assigned
      • Community Habilitation Specialist attends supervision and training as required. It is essential that Community Habilitation Specialist know and protect individual’s rights.
      • Community Habilitation Specialist will document each training session in the appropriate databases and report the progress and barriers with each client.
      • The Community Habilitation Specialist will receive consultation and supervision from the Community Habilitation Supervisor.
