Facilitates home visits with the targeted population.
During home visits, CHCs will assess the needs of the family via a needs assessment survey, this survey will be divided into the categories of the chosen curriculum and program objectives.
The CHC will help the family create a goal for their needs and provide the tools to reach their goal.
Maintains the coordinated care plan for each participant based on needs assessment.
Communicates participant information and needs with the program manager, clinical counselors, and other community health coordinators as needed.
Submits reports regularly as requested by the program manager and counselor.
Follows up routinely on referrals made for each program participant.
Adheres to cultural competency standards and provides every participant with equitable treatment and service.
Other duties as assigned.
High school diploma or G.E.D equivalent
At least one (1) year of Community Health Worker experience
Experience in the public health field and health programming
Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook)
Excellent Communication skills (verbal & written)
Ability to establish and maintain effective relationships and foster teamwork in a diverse environment
Ability to maintain confidentiality when dealing with sensitive information
Ability to work well with the public and staff members
Understands the organizational mission
Understands ethics and public good; is concerned with public trust

Full-Time | Grant-funded position
Monday - Friday | 8 a.m. - 5 p.m.
This position will potentially work evenings and weekends.
This job operates in the community with home visiting needed to establish relationships with the program participants.
Will require periodic travel with some weekend and evening hours.
Persons in these positions frequently lift, carry, push and pull objects weighing up to 10 lbs.
They also occasionally move objects weighing up to 20 lbs.
Harris County Public Health (2223 W. Loop South, 77027) - Will relocate to Downtown Houston by the end of 2022.

Under the supervision of the Maternal and Child Health (MCH) Program Manager, the Community Health Coordinator (CHC) will provide direct family support by administering assessments, resource navigation and referrals, adheres to curriculum guidelines, data entry, follow up, and case management. Enrolled participants will be supported with social and health benefits for new and expecting families through a systematic approach of care coordination, prenatal care, postpartum care, and infant-early child development. Telehealth services may be required in certain circumstances.
