      The Community Health Specialist 2 (CHS 2) - African American KSA position will provide services to pregnant and parenting African American families living in Multnomah county. African Americans have the highest rate of infant mortality, preterm delivery, low birth weight and maternal mortality and morbidity. In this position, you will conduct ongoing screenings for dad, mother and child along with collecting and documenting family health data. The Community Health Specialist (CHS) is part of a case management team and will maintain an individual caseload in consultation with the nurse. Additionally, the CHS 2 will act as a contact for community resources and coordinate ongoing collaboration with community partners. Healthy Birth Initiatives (HBI) utilizes a comprehensive model that includes individual, group and community level strategies and interventions.

      As the Community Health Specialist 2 , you will:

      • Provide African American culturally specific individual and family case management services to HBI clients through regular home visits.

      • Provide prenatal, postpartum and child health screenings; provide health education to clients.

      • Maintain regular communication with families, provide health resources and referral information as needed for health related concerns.

      • Document all interactions, interventions and client conversations in client record; document required data screening forms into the CareWare system.

      • Coordinate with other agencies and health care providers to provide for the psychological and social service needs of assigned families; communicate with health and social service providers as needed to ensure coordinated care and case management functions are being maintained.

      • Build relationships with African American specific and other organizations to build a shared agenda for eliminating health inequities.

      • Meet with other members of the case management team on a regular basis to assess client needs and develop appropriate plans of care.

      • Participate in regular team meetings and case management meetings; serve on internal and external committees and teams as needed; participate in training and conferences as required by the program.

      Minimum Qualifications/Transferable Skills*:

      • Education/ training equivalent to an associate's degree.

      • One (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.

      • This position requires an advanced knowledge and understanding of African American culture, traditions, beliefs and history, including an understanding of the origins and results of the health disparities experienced by African American families related to racism and socio-economic injustices.

      • This position requires strong knowledge of maternal child health, including but not limited to, childbirth, breastfeeding, prenatal and postpartum education.

      • Valid BLS/ CPR certification.

      • A valid driver's license.

      • Must pass a reference check.

      • Deep understanding of disparities that inform equity driven approaches to the work

      Routine Telework: This position is designated as “routine telework,” meaning you will be working remotely (from home); a hybrid schedule also is available. As with all routine telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. All employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations. The onsite work location for this position is Walnut Park Complex, 5329 NE MLK Jr Blvd, Portland, OR 97211.
