What to Expect (Job Responsibilities):
- Foster community organization and resource partnerships to address barriers related to Social Determinants of Health (SDOH)
- Create and implement outreach strategies for recruiting and engaging a diverse Community Health Worker workforce
- Assist in the hiring, training, retention, and onboarding of Community Health Workers
- Train Community Health Workers on identifying, linking, and navigating SDOH resources for justice-affiliated youth
- Collect and analyze comprehensive client outcomes data to identify areas for continuous quality improvement

What is Required (Qualifications):
- High school diploma or equivalent
- Minimum of two years of related community-based experience
- Completion of a C3 aligned Community Health Worker training program
- Bachelor’s degree in Health Promotion, Public Health, Social Work, or a related field is accepted
- Knowledge of local community resources is preferred

How to Stand Out (Preferred Qualifications):
- Higher education or related healthcare training may substitute for required experience on a year-for-year basis
- Experience working directly with justice-affiliated youth or community health initiatives
