A Community Health Worker (CHW) is a public health worker who has a heightened understanding of the community served through their specific program. Depending on clinic priorities or seasons, the specific program the CHW works under may vary. This understanding enables the CHW to serve as a liaison, link and educator between services and the community to promote services and ultimately enhance community health. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, data collection, social support and advocacy. Early Childhood Development (ECD) CHW will be responsible for promoting well visits for children under 5 years old and ultimately supporting families in their success making it to their well visit appointments. The ECD CHW will also assist families needing to complete developmental screening surveys prior to their visit with their provider.
