      Position Title: Community Health Worker II/ Housing case Manager
      Location: Aborn Admin, San Jose, CA
      Employment Type: Full-Time

      Overview

      The Community Health Worker II (CHW II) plays a vital role in supporting clients by providing comprehensive case management, resource coordination, and housing-related assistance. This position involves direct client interaction, document collection, data tracking, and collaboration with landlords and property managers to ensure clients’ needs are met effectively and efficiently.

      Key Responsibilities

      • Case Management: Provide individualized support to clients, including enrolling clients in programs, conducting assessments, and developing care plans to address their needs.

      • Resource Coordination: Connect clients with community resources, including housing, healthcare, and social services, to promote stability and well-being.

      • Housing Support: Assist clients with housing-related needs, including completing housing applications, conducting home visits for inspections, and processing paperwork.

      • Document Collection & Processing: Collect and verify client documents, complete required forms and assessments, and process check requests for housing or other financial assistance.

      • Landlord Communication: Engage with landlords and property managers to secure housing, clarify lease terms, and coordinate document submission.

      • Data Tracking & Documentation: Maintain accurate client records, track service delivery, and update case management databases to ensure compliance with program requirements.

      • Home Visits: Conduct home visits to perform housing inspections, deliver services, and complete necessary paperwork.

      • Office Support: Perform administrative tasks, including filing, data entry, and other office-related duties as needed.

      Qualifications

      • Education: High school diploma or equivalent required; associate’s or bachelor’s degree in social work, public health, or related field highly preferred.

      • Experience: Minimum of 2 years of experience in case management, community health, or social services; experience with housing programs is a plus.

      • Skills:

        • Strong interpersonal and communication skills for engaging with clients, landlords, and stakeholders.

        • Proficiency in documentation, data entry, and case management software.

        • Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.

        • Knowledge of housing resources, community services, and program compliance requirements.

      • Other Requirements:

        • Valid driver’s license and reliable transportation for home visits.

        • Ability to maintain confidentiality and adhere to HIPAA and program privacy policies.

        • Bilingual skills (e.g., Spanish, Vietnamese, or other languages) are highly desirable.

Under the general direction of the supervisor/manager, the Community Health Worker performs a variety of services. Duties include:

      • Link patients to services within BACH and the broader community.
      • Participate in staff team meetings, case conferences and training.
      • Collect data and provides documentation in client files as required by agency and funders.
      • Participate in obtaining deliverables required for various grants, such as, but not limited to, eligibility assessment, socioeconomic needs assessment, and medical disparities.
      • Assist families with immediate crisis intervention, advocacy, problem-solving and other appropriate interventions, as needed
      • Utilize de-escalation techniques in situations as appropriate
      • Utilize program evaluation tools (i.e., pre/ post-tests, satisfaction surveys, etc.), analyzing and reporting on outcomes
      • Provide resources for various health topics, such as nutrition, physical activity, hygiene, sexual health, and substance abuse.
      • Maintain accurate and confidential records of interactions with students, families and other stakeholders.

      Secondary Responsibilities

      • Attends workshops, training, and meetings as needed, and as requested.
      • Performs other duties as assigned by supervisor.

      Qualifications


      Qualifications:

      Required Education, Experience, and Training

      • High school diploma or equivalent
      • 1-2 years of experience in a community-based health care service capacity preferred
      • Bilingual preferred
      • CHW Certification - Recertification and/or retraining process, as required
      • CEC Certification

      Work Conditions

      • Travel to other sites and/or locations required
      • May visit patient in their homes or accompany patients to their appointments
      • Work outside in varying temperatures may be required
      • Prolonged periods of standing or sitting.

      Physical Requirements

      This position requires:

      • Sitting, walking, and standing associated with office environment
      • Regular bending, stretching and lifting
      • Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
      • Traveling to work sites may include exposure to outside elements
      • Presenting information in front of a group or individual
      • Reading and writing in order to review records for accuracy
