      Job summary

      If you are looking for an exciting and challenging job why not join the Community Urgent Community Response Team working in the West of Norfolk. An excellent opportunity has arisen to enhance our team of Experienced Health Care Assistants to provide delivery of care for patients in their own home with long term conditions, palliative care and rehabilitation needs across West Norfolk. Also to provide enhanced care to patients in their own homes up to 72 hours or until infection has reduced.

      You will be working within a team to facilitate early discharge from the acute setting and also admission avoidance. Referrals also received from Community Matrons, General Practitioners.

      You will need to be able to demonstrate effective communication skills, problem solving skills and be resilient and able to cope in changing and challenging situations. You will work under the direction of Registered Nurse but will also be required to work autonomously. Previous experience in health care as a Health Care Assistant in community setting would be desired but not essential.

      The Urgent Community Response service runs from 08:00 - 22:00 seven days a week, it also covers night sits from 22:00- 07:00 seven days a week.

      Main duties of the job

      To assist in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community, community hospitals and in clinics, by following prescribed packages of care.

      • To work within the supported care team to prevent unnecessary admission to hospital.
      • To make risk assessments, establishing the safety or otherwise of a patients ability to stay at home, or return safely from another care setting, providing and arranging as necessary supportive packages of care.
      • To provide in-reach to inpatient units and step up beds where required.
      • To act as the patients advocate, facilitating choice, patient empowerment and independence.
      • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
