- Deliver high-quality personal and clinical care to patients in the community
- Support individuals with chronic conditions, post-hospital care, and patients at end of life needs
- Working autonomously but will have the support and supervision of qualified District Nurses

- Level 3 Diploma/NVQ in Health & Social Care or equivalent experience delivering care at this level
- Additional clinical skills training (e.g. wound care, phlebotomy, catheter care)
- Care Certificate (or willingness to complete during onboarding
- Basic clinical care skills (e.g. observations, assisting with hygiene, continence, nutrition, mobility)
- Able to follow care plans and report concerns
- Accurate and timely record-keeping
- Ability to work independently and within time constraints
- Awareness of health promotion, reablement, and long-term condition support
- Ability to support behaviour that challenges or complex needs
- Experience providing hands-on care in any setting: community, hospital, domiciliary, or unpaid (e.g. family care)
- Experience supporting people with long-term conditions, frailty, or disability
- Experience working in a community setting or supporting patients in their own homes
- Experience supporting older adults or palliative care
- Ability to use MS Office e.g. Word, Excel, Outlook
- Able to maintain accurate records using electronic systems
- Experience using EMIS or similar clinical systems

Job summary
Community Healthcare Support Worker (HCSW) - Band 3 Salary: £29,176 - £30,225 (inclusive of HCAS)Location: Across the London Borough of Bromley - Bromley, Beckenham, Orpington & Biggin Hill
Hours: Ideally FT & some PT (minimum 3 days/week) availablePlease note: The first 6 weeks of the role will require full-time attendance for training.
The post-holder is required to work across a 7-day rota (7.5 hrs per day), including weekends and bank holidays, to meet the needs of the local community.
Times are a mix of 8.15am - 4.15pm & 10.45am - 6.45pm.
