- Develops, implements, manages and maintains operational systems and activities.
- Serves as a liaison with all levels of the organization and outside community.
- Manages multiple priorities and projects with competing deadlines.
- Allocates time to meet completion requirements. Serves as a coach and mentor for other positions in the department.
- Reviews and monitors adherence to Human Resources policies and corporate compliance procedures.
- Other duties as assigned.
- Develop a working knowledge of existing conditions, trends, activities and public policies affecting the community, and assists with assessment of health status and conditions within the social environment
- Leads in creation and approvals of Community Health Needs Assessments within assigned geographic area; leads updates to market leadership\board on status of implementation plans
- Implement priorities and measure outcomes from local Community Health Needs Assessments
- Develop effective relationships with community leaders and residents across diverse interests, cultures and socio-economic backgrounds, including formation of community coalitions and priorities
- Support the coordination of local committees and local implementation programs or projects related to the priority initiatives of the Health Anchor Network (HAN)
- Oversight of Community Benefit reporting activities
- Collaborate with and maintain network of civic partners in the service of addressing the health and well-being needs of communities
- Serve as trusted resource to local and market leadership teams in the promotion of effective community partnerships across the care continuum, providing direct management to key resources as assigned; Aligns with Mission, Diversity & Inclusion, Population Health, Advocacy\Government Relations, Marketing\Communications, etc
- Prepare internal and external documents, including evaluation and progress reports, board and public presentations, and funding requests
- Bachelor's degree in Business Administration, Healthcare Administration or related field.
- Associate’s degree and five years of experience may substitute the Bachelors degree requirement.
- Master’s degree and two year of experience may substitute for one year of experience.
- May be required for specific positions as per policy or regulation.
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to take appropriate action in a stressful environment.
- Demonstrated team, leadership, organizational and problem solving abilities.
- Three years of experience in project management, data analysis and systems training.
- Data analysis experience in a healthcare setting preferred.
- Experience working in the public health environment
- Has good understanding of public health equity and the social determinants of health
- Statistical analysis experience
- Experience speaking with key stakeholders
- Ability to perform work in a stationary position for extended periods
- Ability to operate a computer and other office equipment
- Ability to communicate and exchange accurate information
- Ability to travel throughout the hospital system
- Follows Standard Precautions using personal protective equipment as required for procedures.

As a Program Manager II, you will oversee and coordinate different related projects to ensure they benefit one another and meet business goals.
Some of your duties will include but are not limited to:
• Managing implementation and maintenance of designated programs.
• Planning, Developing and Maintaining systems and procedures.
• Serving as a coach and mentor for other positions in the department.
The Future Caregiver is someone who:• Has High-level management and leadership skills.
• Can schedule and manage tasks effectively.
• Has a public health background (preferred).
• Maintains and applies a broad understanding of financial management principles
• Has experience working with/ supporting underserved populations/Social Determinants of Health.
