      Job summary

      We have set up a new community rehabilitation team to support people with serious mental illness and complex needs. The community mental health rehabilitation team will offer a 7 day service for people living in North Lincolnshire. The service will enable people to receive an intensive community rehabilitation service closer to home in their own community.

      The community rehabilitation service will consist of a multi-disciplinary, multi-agency workforce, working collaboratively between Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) and North Lincolnshire Council to share skills and specialisms to support individuals and improve their quality of life.

      This is an exciting opportunity for an experienced Registered Mental Health Nurse (RMN) to join our team as a Band 6 Community Mental Health Nurse. The right candidate will be enthusiastic and a highly organised mental health professional who is able to provide high quality nursing care, assessment and treatment.

      The successful candidate will provide a comprehensive Community Mental Health Nursing Service across a whole range of clients experiencing mental ill health who are engaging in the community rehabilitation pathway. The service will be delivered in collaboration with representatives from other agencies and therefore the ability to work independently and with partner agencies is essential.

      Main duties of the job

      It is essential that applicants can demonstrate post registration experience as an RMN, which may include inpatient and community settings.

      The following tasks and duties will also form part of the role; however, this list is not exhaustive:

      To provide a clinical service that is patient focused, accessible and responsive over a 7 day week

      To provide and deliver highly skilled nursing care, including assessment, care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management and promote recovery and inclusion

      To ensure that medicines are administered in accordance with the Trust Policy and legal requirements

      To undertake clinical procedures, training and supervising junior staff in these procedures, for the care and welfare of patients

      To maintain a high standard of professionalism and to establish a good and effective working practice, in accordance with NMC professional practice guidelines

      Contribute to the prevention and control of infection through standard infection control practices and compliance with the Trust infection control policies to ensure the highest quality of care to patients

      To respect the views of patients, carers, families and significant others and work collaboratively with them when planning care and assisting them to meet their needs
