      About the team:

      Lewisham's Community Services are undergoing a radical transformation Programme and an exciting opportunity has arisen to take on the role of Mental Health Practitioner for our new Community Mental Health Team Neighbourhood 4.

      The transformation will improve access to our services and will build much closer relationships between secondary and primary care within the local neighborhoods.

      The post holder will work as a mental health practitioner for a caseload of service users in the local community who have complex mental health difficulties, and work with their carers where applicable.

      The post holder will be expected to have excellent self-management skills, and also work collaboratively within the team. The post holder must be able to formulate flexible, robust and holistic care plans with service users and/or carers, to work well under pressure, be experienced in dynamic risk assessment, and also to have excellent communication skills.

      We would also welcome applications from nurses, Social Workers who have significant experience and who are looking to develop new community skills and expertise in managing adults with mental Health issues.

      About the role:

      We are looking for a highly experienced and enthusiastic Mental Health practitioner with a hands-on approach who relishes a challenge, has a real passion for mental health and who models the highest standards of care in their practice.

      This is a varied and challenging role and requires an experienced mental health professional (nurse, OT or social worker) with knowledge of working with service users with severe and enduring mental health problems. You will lead a diverse multidisciplinary mental health team integrating staff from health, social care and thevoluntary sector. The team offers a wide range of focussed interventions to service users with a range of diagnoses, and to their carers.

      You will be supported through regular supervision and access to training. We are committed to helping to develop the people who come to work with us.
