Job Description

- Coordinate services with relevant community resources, service providers, and outside agencies, to ensure a holistic approach to residential needs and outreach
- Assess resident and community needs with comprehension and critical thinking
- Monitor and follow-up with residents and individuals progress to obtain direction and referrals for services and evaluate the effectiveness of the interventions
- Provide and collaborate with key resources and assistance necessary at the OneStop facilities and radius communities
- Communicate and advocate to serve internal and external partners and stakeholders who work with the OneStop 4 Help and its strategic program operations
- Provide education and outreach through individual, group, and community efforts to raise awareness and engagement
- Maintain accurate and up-to-date records, progress notes, and documentation of services provided
- Maintain ethical consideration, cultural competency, and professional boundaries to provide excellent customer service and sensitive interactions.
- Remain flexible and communicative in office, field, and other resident interactive settings that inform, help, and sustain resident request and need.
- Educate and provide information about programs, services, and opportunities to the public at community events, resource and job fairs, and other forums as needed
- Develop and engage in building relationships through outreach with groups, agencies, organizations, and resident-related interactions
- This job requires fingerprinting.
- This job may require on-call rotation and emergency after hours, weekends, and/or holidays.
- Bachelor's Degree in social services, social work, sociology, public health, human services, or psychology
- One year of experience in case management
- Or, an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as listed above
- Valid driver's license
- CPR certificate
- First aid certificate
- Two years of experience in case management principles, applicable laws, rules, and regulations
- Montreal Cognitive Assessment (MoCA) or other certification or training in social work, or other social services
- Experience in a high-capacity social work or human services operations
- Experience in community outreach, community resource or community support
Job Description
The Community Navigator is responsible for providing case management to citizens and incumbents, conducting assessments, providing counseling to individuals and families, and maintaining reports and records. They are also responsible for p roviding the community relationship development with health, human, and social service community partners to best encompass the needs of a site. They s erve as the resource representative in each district area, to help navigate needs and issues, such as food insecurity, homelessness, emergency housing, utilities, and rental assistance. They i dentify and collaborate with community partners to address needs through active communication, decision-making, and problem-solving.

The grade for this position is B241.

The County Administrator and County Attorney are appointed by the Board of Commissioners and serve at its pleasure. To implement the Board's policy directives, the County Administrator uses a management team consisting of members of his immediate staff and 14 department directors. The 14 departments that make up the executive side are Police Services, Fire and Emergency Services, Corrections, Communications, Support Services, Financial Services, Community Services, Human Resources, Information Technology Services, Law, Planning and Development, Water Resources, Child Advocacy & Juvenile Services, and Transportation. Each department is run by a director who is charged with managing operations in a manner which stresses the County’s values.
In addition to the executive side of County government, certain services are provided through Constitutional officers and independent elected officials. These external offices are created by the Georgia Constitution or through state law. In Gwinnett, they include the Sheriff, Tax Commissioner, District Attorney, Solicitor, Probate Court Judge, and Chief Magistrate.
