- This role provides prompt, efficient co-ordination and administration support to community nursing services.
- You will be the main point of contact for service users, their families, other teams and organizations.
- Promptly respond to incoming referrals from various sources, including : SystmOne, email and telephone.
- The role will require effective communication skills via various methods with the ability to adapt this to support service users with complex needs.
- You will be required to co-ordinate interventions by prioritising and allocating Care Plans to appropriate team members referring to Standard Operating Procedures and escalating where appropriate to the Senior Nurse on Duty.
- The aim is to support the clinical team to deliver high standards of care by ensuring continuity of care and co-ordinating the service.
- GCSE passes at Grade C or above or equivalent experience
- Evidence of consistent pattern of learning from education, training and experience
- Understanding of basic human needs - physical, emotional, health and social
- NVQ level 3 or equivalent (health and social care or business administration)
- IT/Administration Qualification e.g. ECDL
- Knowledge of assessments & Care
- Management Process
- Knowledge of medical terminology
- Knowledge of social care & health systems, processes and pathways
- Significant experience of working in a health or social care setting
- Previous administration experience
- Working with multi-disciplinary teams
- Experience of working with vulnerable people
- Considerable experience needed in working with the public
- Use of recognised IT packages e.g. Microsoft Office, Excel and Access
- Effective keyboard/IT skills, which include accurate recording skills
- Ability to undertake statistical and data analysis
- Proven interpersonal skills and appropriate empathetic and tactful manner
- Good communication skills to communicate effectively with customers, careers, specialist services and colleagues
- Ability to meet conflicting deadlines, prioritise effectively and manage own workload
- Good organisational skills with the ability to use time effectively and prioritise work
- Self-awareness of own strength and limitations
- Demonstrate an ability to undertake duties with a degree of autonomy and accountability
- Effective working as individual but also as part of larger team
- Evidence of continued personal learning and development
- Willingness to undertake training to develop within the role
- Effective in using multiple database computer systems e.g. S1
- Customer care/reception training
- Evidence of risk assessment training/awareness
- Flexible working

An exciting opportunity has arisen to join the forward-thinking and dynamic PCN community team in Crowborough, Ashdown Forest, and Wadhurst catchment area.
This pivotal role will provide prompt, efficient co-ordination and administration support to PCN teams, playing a significant part in the day-to-day administration of the work in relation to Community Nursing.
