Conduct in-person visits in the patient’s home setting to engage individuals who are overdue for care or at risk of being lost to follow up.
Provide exceptional, compassionate customer service.
Establish contact with the patient by checking and aligning their demographic information in the patient tracking system.
Engage patients in conversation to assess barriers to care and develop tailored re-engagement strategies.
Schedule an appointment with Village Medical PCP, High Risk Clinic and/or Village Medical at Home, or encourage real-time telehealth visit while on-site.
Provide hands-on support for telehealth technology, including setting up smartphones, tablets, or other devises; installing necessary apps; and assisting with identifying reliable internet connectivity.
Educate patients and caregivers on how to use telehealth platforms and remote monitoring tools effectively and confidently.
Coordinate closely with primary care teams and high-risk programs to ensure a smooth continuum of care.
Identify and escalate social, clinical, or technical issues to appropriate team members.
Maintain patient confidentiality and always adhere to HIPPA regulations.
Represent VillageMD with empathy, professionalism, and culture sensitivity in every patient interaction.
Strong interpersonal communication skills including active listening and empathy to build trust with patients.
Cultural humility and sensitivity when working in diverse home environments.
Ability to motivate patients to schedule appointments and reengage in their healthcare.
Comfort with complex technology including mobile devices, apps, video conferencing tools and remote monitoring devices.
Ability to troubleshoot basic technology issues (wi-fi, device set up, app navigation)
Teaching others how to use technology with patience and clarity.
High School Diploma/GED required, Bachelor’s Degree preferred
At least one year experience in sales, client relations, or community relations required; prior experience in healthcare preferred
Customer service mentality and desire to help patients solve problems
Proficient PC skills with strong written and public communication skills
Familiarity with customer relationship management (CRM) systems preferred
Strong interpersonal, organizational, and time management skills
Access to a vehicle that you can use every day in job functions. Mileage is reimbursed.
Comfort with ambiguity and ability to proactively structure work and problem solve independently
Highest degree of honesty, integrity and fairness
Fluency in Spanish to serve patients in the communities we operate in preferred
Experience in healthcare preferred

VillageMD is looking for a compassionate, tech-savvy, and highly motivated Community Outreach Specialist. This role is designed to bridge the gap between patients and their care by conducting home visits and assisting with technology enablement to support virtual care services. You will work directly with patients who are disengaged, or at risk of falling out of care, to provide technical support and re-establish trust in healthcare systems. Your primary responsibility will be to connect with VillageMD’s attributed patient population by phone, email, or in person. This position is ideal for someone who thrives on making personal connections, problem-solving in the field, and empowering individuals through healthcare access
