      Join Our Team as a Community Palliative Care Specialist Nurse! We are based at Unit 16 Building Research Establishment, Watford

      Are you passionate about making a difference in the lives of patients with palliative and end-of-life care needs? We have an exciting opportunity for you to join our Specialist Palliative Care Service at the Central London Community Healthcare NHS Trust - Hertfordshire Division.

      As a Community Palliative Care Specialist Nurse, you will be an integral part of our integrated Cancer, Non-Cancer, and Palliative Care team, working across organizational boundaries to deliver exceptional care. Your role will encompass:

      • Advanced Clinical Practice
      • Training and Education
      • Service Development/Leadership
      • Audit

      Main duties of the job

      We are looking for a compassionate and dedicated nurse who is:Qualified with experience in palliative/end-of-life care

      Organised and flexible, able to adapt to the needs of our patients

      Compassionate towards patients and their families

      Collaborative, with experience working in a multidisciplinary team

      Proficient in IT, with experience using SystmOne

      Excellent in communication, with strong community nursing skills

      Skilled in triaging to ensure the best outcomes for patients

      Holder of a full, valid UK driving licence and access to a car for business purposes (unless exempt under the Equality Act 2010)

      Provide highly specialist palliative care advice and support to other generalist nurses, care agencies and health professionals including doctors and consultants, to ensure high quality individualised patient care.

      Use expert skills in communication, clinical judgement and management within palliative care settings.

      Function effectively as an autonomous practitioner working independently not directly supervised and as a member of a multidisciplinary team.

      • First level degree or undertaking a first level degree
      • Teaching qualification or willing to undertake
      • Advanced Communication Course
      • Do Not Attempt Cardiopulmonary Resuscitation competencies
      • Willingness to undertake further academic or practical study at a higher level for e.g. Master's degree

      Desirable

      • Extended Nurse Prescribing Qualification

      Experience

      Essential

      • Candidates should have attained an appropriate level of experience to the fulfil the competencies required in palliative care or oncology
      • Suitable applicants will have demonstrable post registration experience working at a senior level in palliative or end of life care
      • Community experience
      • Evidence of experience in a teaching and mentoring role
      • Experience of cross boundary working in partnership and collaboration with health, social and voluntary sectors
      • Knowledge of NMC code of professional conduct
      • Understanding of current political and policy context in Primary health Care
      • Knowledge of research processes
      • Knowledge of clinical governance and its implications
      • Knowledge of resource management
      • Up to date evidence of continued professional development and of reflective practice in a professional portfolio
