      Job summary

      Seisdon Primary Care Network are looking for a dynamic and enthusiastic paramedic to join our Primary Care Network. Working Mon-Frid 5 days a week the role will work closely with our North practices (Perton, Codsall, BIlbrook and Featherstone). We have a close and expanding team of ARRS clinicians serving around 50,000 patients in Perton, Codsall, Bilbrook, Featherstone, Wombourne and Claverley. Our practices provide the successful candidates a supportive environment to develop and grow.

      Main duties of the job

      Undertake home visiting for housebound patients and those patients residing in Care Homes

      Assess patients presenting with acute illness or for review of long term chronic diseases.

      Assess and triage patients, including same day triage and as appropriate provide definitive treatment or make necessary referrals to other members of the primary care team.

      Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

      Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.

      Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs

      Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

      Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

      Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.

      Perform specialist health checks and reviews; perform and interpret ECGs; perform investigatory procedures as required.

      Job responsibilities

      Provide an alternative model to urgent and same day GP home visit for the network and undertake clinical audits.

      Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered.

      Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.

      Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice

      Other Duties

      Work within the policies of the Practices, PCN and GP First.

      Promote client involvement in the management of the service.

      Participate in regular supervision sessions and appraisals with your line manager. Attend training and development activities as identified and participate in meetings as required.

      Maintain administration systems and workspace used in a clean and tidy condition.

      Maintain a good working knowledge of Health and Safety procedures and fire precautions, and operate the correct procedures and participate in policy development and data collection where appropriate.

      Work flexibly to meet the needs of patients

      Undertake any other duties that are commensurate with the post and within the post holders clinical competence.

      • Completed a two year Consolidation of Learning period as a newly qualified Paramedic
      • Further three years experience as a band 6 (or equivalent) paramedic
      • Has Level 7 capability in paramedic areas of practice
