      Job summary

      Tired of the acute hospital focus to clear beds? Be part of the Community Rehab service in Somerset as a Community Rehabilitation Physiotherapist working as part of a Multi-disciplinary Team, alongside other Physiotherapist, Occupational Therapist, Rehab Assistant and Pharmacy Technician colleagues. Our aim is to work with people on what really matters to them to enable them to maximise their abilities and improve their quality of life through skilled rehabilitation and a focus on embedding self management strategies and behaviours, so they live well with their conditions.

      We have an emphasis on prevention and early intervention at the initial point of need but also on long-term self-management and linking patients to other resources available to them in their community. We see people in a host of settings including their own homes and local community, our community hospitals and our local care homes.

      A Band 5 post has arisen for a Physiotherapist to work within the Taunton and West Somerset Community Rehabilitation Team. You will play an integral part, managing, with support, a complex community case-load. These patients may present with a broad range of conditions including orthopaedics, frail elderly, post-surgery and neuro long-term conditions.

      Main duties of the job

      To provide high quality patient-centered care and specialist rehabilitation to adults referred to the Rehabilitation Service.

      Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments, where there are teams supporting hospital inpatient units

      To provide advice regarding the rehabilitation management of patients to families, carers and other members of the MDT.

      Responsibility for a caseload of patients within both the community and/or the community hospital bedded environment.

      Carry out duties autonomously but with the ability to seek support and informal and formal supervision from Senior Therapists within the teams.

      Alongside Senior clinical staff and Clinical Educator, give support, guidance and training to Rehabilitation Assistants, Assistant Practitioners, Apprentices and Students on placement within the team.
