      Job summary

      We are currently seeking a Community Sister to join our Easington South Community Nursing Team. The successful applicant will work across the easington locality and the surrounding areas from an office base in Peterlee. The team cover 8am until 9pm 7 days a week on a duty roster.The post holder will be a caseload holder to ensure the provision of clinical management to District Nursing and Intermediate Care Services. The role involves providing holistic nursing care to adult patients in their own home or residential care setting. Successful applicants will be required to hold a first level nursing registration with the NMC, additional training specific to the role can be provided.

      Main duties of the job

      To support coordination and delivery of high quality nursing care that, where possible, meets identified patient need, supports the aims and objectives of the integrated health and social care team, and remains within the boundaries of available resource. Predominantly complex clinical care and caseload management with some coordination and supervisory responsibilities for health care staff between Bands 2 to Band 5. The Community Nursing Team provide both planned and reactive care including Urgent Community Response.

      We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

      • Non-Medical Prescribing
