The Community Social Worker promotes independence and quality of life for all residents of Pilgrim Place to lead a fulfilling, active, purposeful, and secure lifestyle. S/he models, implements, and coordinates positive patterns and structures of person centered care (PCC) across the campus.

This position fulfills two creative and demanding roles:
-
Casework: Assists residents in independent and assisted living to plan and implement life transitions in housing and activities of daily living as their physical and mental capacities diminish.

Group work and networking: Coordinates staff and residents to implement the Pilgrim Place commitment to PCC and the ongoing culture change that it requires. While its primary focus begins within the Health Services Center (HSC), this effort will involve all staff, all residents and their health support groups, and Board members.

Effectively contributes to a warm and supportive environment by communicating and responding to a diverse and active resident population and answering pertinent questions from families and responsible parties in a timely and clearly understandable manner.

Assess residents for health, psychological and social service needs; monitor their capabilities to reside safely in their existing homes; facilitate housing transitions as necessary.

Provide direct social services for residents and their families to help with personal as well as external and environmental matters.

Successfully negotiates medical settings and coordinates services with professionals, staff, residents and their family members;

Maintain accurate records and report on residents’ status.

Make referrals and act as liaison between qualified health care providers, community resources and campus services on behalf of resident members.

Monitor and assess the quality and effectiveness of service delivery to residents; ensure that all referrals are reviewed and responded to in a timely manner (may include after hours and weekend work).

Develop and maintain a directory of available community services and resources to share with residents, their families, and staff.

Model and coordinate shared leadership as a proponent of the principles of culture change and PCC across the campus and with local allies;

Meet regularly with staff at the HSC and Pitzer Lodge to empower them to use their own insights and skills; to help grow their sense of teamwork; to collaboratively propose, evaluate, and refine new initiatives in PCC and set and evaluate PCC goals.

Develop creative individual and group program plans for PCC and supervise their implementation.

Consult with nursing staff and residents as they deal with challenges as caregivers.

Responsible for follow-through on Action Pact trainings and coordinating implementation of the Centers for Medicare Services (CMS) proposal plan (e.g., collaborate with nurse leaders to complete the transition to face-to-face HSC neighborhoods with active sharing circles and Resident Councils).

Delegate recordkeeping about PCC initiatives; compile reports as required by CMS grant funding sources.
- Demonstrates a passion for working with seniors to improve their quality of life.
- Has knowledge of casework processes, especially needs/strengths of geriatric populations.
- Skilled in collaborating with outside agencies/resources to connect people with relevant services while offering support and guidance along the way.
- Has working knowledge of funding sources such as Medicare and other insurances.
- Skilled in gathering and facilitating working groups; networks effectively with people in a multi-cultural environment.
- Excellent written communication, organization, and time management skills; proficient in using Microsoft Office products.
