- Working very closely with our clinical and wellbeing teams and external agencies
- Be the initial line of support for patients and families, providing advice, signposting services, considering advance care planning, and providing support in times of distress.
- Provide the point of access for other health and social care professionals providing relevant end of life support and advice as well as signposting education opportunities.
- Registered General Nurse.
- First-level degree or demonstrate equivalent experience and skill
- Post-registration qualification in cancer/palliative care or relevant subject or working towards
- Evidence of continued professional development
- Teaching qualification
- Communication skills training
- Relevant post registration experience in cancer, palliative care, or another speciality.
- Experience of multi-professional working
- Evidence of good communications skills both face to face and over the phone
- Ability to support and influence change
- Evidence of solid IT skills and knowledge
- Demonstrate ability to meet Oakhaven Hospice values
- Demonstrate advanced clinical expertise
- Experience of being involved with sustainable change
- Evidence of understanding professional issues in nursing

Job summary
We are seeking to recruit a Registered Nurse to join our community team which includes supporting our new Single Point of Access Service (SPA). The role will be supporting our patients and families, providing both telephone and face to face advice. The aim is to enhance and support palliative and end of life care within our catchment area.
