Job summary
An exciting opportunity has arisen to join the community nursing service in Halton. Are you an enthusiastic and motivated nurse looking for a new challenge? You will join a team of nurses delivering high quality care to patients who are housebound. This includes palliative care, wound care, catheter care and phlebotomy. If you feel you have the right skills to care for patients in their home environment, then we would like to hear from you. Main duties of the job

To competently perform clinical skills related to the post.
To be professionally responsible for a delegated caseload under the supervision of the District Nurse.
To assess health needs at individual, practice and community level.
To undertake chronic disease management reviews using appropriate template.
To work without direct supervision.
