- Deliver Skilled Nursing Care: Provide high-quality, evidence-based nursing care, ensuring best practice principles are followed.
- Comprehensive Patient Assessment: Conduct thorough assessments of patients' physical, psychological, and social needs, including carers' requirements. Develop individualised, person-centred care plans that promote self-care and help prevent unnecessary hospital admissions.
- Care Planning and Continuity: Under the supervision of the District Nursing (DN) Team Lead and Community Nursing Sister, take ongoing responsibility for the assessment, planning, implementation, and evaluation of holistic healthcare needs. Ensure continuity of care following referrals from acute settings, GPs, carers, and other agencies.
- Problem Identification and Escalation: Identify and document new problems or changes in condition, reporting findings to the Band 6 nurse as appropriate.
- Accurate Record Keeping: Maintain accurate, comprehensive, and contemporaneous electronic records in line with Trust and NMC guidelines. Full training on the use of electronic record systems will be provided.
- Liaison and Referrals: Liaise effectively with GPs, other services, and agencies, ensuring timely and appropriate referrals to optimise patient care.
- Patient and Carer Support: Provide information, guidance, and emotional support to patients, families, and carers to promote positive health outcomes.
- Autonomous Practice: Ability to work independently, managing your own caseload and making informed clinical decisions.
- Diabetic Management: Blood sugar monitoring, insulin administration, with access to a specialist diabetic nursing team.
- Tissue Viability/Wound Care: Leg ulcer management, compression therapy, doppler assessments, VAC dressings, pressure ulcer care, and post-surgical wound management. Supported by a specialist tissue viability team.
- Bladder and Bowel Care: Male, female, and suprapubic catheterisation, bowel care, and continence assessments.
- IV Medication Administration: Care of PICC/midline, preparation, and administration of IV medications.
- Palliative Care: Symptom management, support visits, administration of anticipatory medications (stat doses and syringe drivers), with access to a specialist community palliative care team.
- Drain Care: Management of JVAC, lantern drains, and Pleurex drains.

Job summary
We are seeking a compassionate and motivated Community Staff Nurse to join our dynamic and supportive Yateley community nursing team. This exciting role is a fixed term post for a year.
This post involves delivering high-quality nursing care to patients in their own homes, offering variety and autonomy in your daily practice.
As a Community Staff Nurse, you will provide holistic nursing care, supporting patients with a range of conditions, including wound care, diabetic management, catheter care, and palliative care. You will work closely with a multidisciplinary team, accessing specialist support as required to ensure optimal patient outcomes.
