      Job summary

      The Team provides specialist assessment and support for people who are presenting with possible onset of Dementia and/or a diagnosis of Dementia. You will work as part of a multi-disciplinary team providing person-centred support to improve the quality of life for people living with a dementia, their family and carers and be committed to the concept of living well with Dementia.

      We are offering an exciting opportunity for a Community Support Worker who is passionate about working with people living with a dementia to join our service working in our Dementia Home Treatment Team. You will need to hold excellent communication skills, be enthusiast and flexible in your approach.

      As part of our service you will provide signposting and support via planned interventions to people living with a dementia in the community in accordance with an agreed care plan and under the guidance of a qualified practitioner. This role is varied allowing plenty of scope to both demonstrate and develop your skills. You will work closely with and be supported by our multi-disciplinary team having access to the support of admin, fellow support workers, psychologists, consultant psychiatrists, nursing and occupational therapy staff. Whilst working alongside the immediate team you will also work with our wider health and social care community, in particular our colleagues in general practice, care homes, social care, admiral nurses and our 3rd sector services such as the Alzheimer's Society.

      Main duties of the job

      • Ensure the delivery of high-quality care in the Dementia Home Treatment Team and demonstrate high personal standards in clinical practice.
      • To ensure care is provided in line with the National Dementia Strategy, associated NICE Guidance and other relevant documentation and that there is knowledge of national, regional and local drivers underpinning service delivery including the Mental Health Act 1983 (revised 2007) regulations & the Mental Capacity Act (2005)
      • With support and supervision participate in procedures for prioritising work and caseload management.
      • To participate where appropriate in robust risk assessments and risk management plans under the supervision and guidance of the lead professional and support the team to practice positive risk taking where appropriate to improve quality of life, well-being and maximise occupational performance/independence.
      • Under the supervision and guidance of the lead professional to deliver a range of non-pharmacological interventions following specific assessments to enable understanding of any expression of an unmet need and to maximise an individual's abilities.
      • Provide signposting and support to an identified group of people living with dementia via planned interventions, in accordance with an agreed care plan and under the guidance of a qualified practitioner
      • Work flexibly across the countywide service, dependent on skill and service need
