      Job summary

      We are looking to recruit new Community Therapy Support Worker to join our friendly Therapy team in Erewash.

      We have part time hours available (3 days per week), giving opportunity to work within a variety of our different workstreams to include Higher Priority and Routine core.

      You will be required to see patients in the Community which will give you the opportunity to adopt agile working between your own home and our two bases which include Long Eaton Health Centre and Ilkeston Community Hospital. This offers great flexibility and independence for diary day to day.

      The Core Community Therapy team comprises of Physiotherapists and Occupational Therapists with band 4 and band 3 support workers.We are part of a large Integrated Community Team which include Community Nursing, Care Co-ordinators and the Erewash Care Team.

      We work closely with Rapid Community Therapy and you will be required to work weekends as part of a shared rota. This will give you the opportunity to see urgent patients as part of a wider team. You will receive enhanced weekend pay and days off in the week as a result.

      Main duties of the job

      You will be required to follow treatment plans within the patient record and under the direction of the qualified Therapists, deliver Therapy intervention to patients in the Community setting.

      You will have a set diary structure for patient visits (6 per day) and you will be expected to manage your diary accordingly. You will also be expected to attend and engage in daily morning capacity meetings and week MDT meetings.

      We like to view Community as a speciality in it's own right and you will be exposed to a range of conditions and scenarios which include Falls, MSK, Neurology, Respiratory, manual handling and equipment provision. This gives you the opportunity to develop your knowledge and skills across a wide area with an underlying focus on Frailty.

      You will be required to do an array of mandatory training related to your role and we will identify any gaps in your knowledge and skills and support you to close these to gain your competencies as a Community Therapy Support Worker.
