What to Expect (Job Responsibilities):
- Act as the primary care manager for high-risk members with complex medical and behavioral health needs
- Conduct comprehensive needs assessments to develop individualized care plans
- Collaborate with internal care teams and community resources to implement care plans
- Provide referrals and linkages to appropriate services for members
- Engage members face-to-face and telephonically to support their health and quality of life outcomes

What is Required (Qualifications):
- Possess a Master's degree in social work or a related field OR a Bachelor's degree with 1+ years of case management experience
- Current, unrestricted licensure as a Registered Nurse in Indiana or LPN with 3+ years of clinical experience
- Experience working with behavioral health providers and community support organizations
- Proficiency in MS Office applications
- Resident of Indiana with reliable transportation for travel within assigned territory

How to Stand Out (Preferred Qualifications):
- 2+ years of experience in long term support services or working with older adults
- Background in Managed Care
- Experience in team-based care
- Additional training or certifications in relevant areas such as Motivational Interviewing or Trauma-Informed Care
- Bilingual in Spanish or other languages specific to market populations
