District Wide - Support student achievement ensuring delivery of 504 plan of instruction according to the individual students.
- Ensure student progress is evaluated on a regular, systematic basis, and findings are used to make decisions to improve program effectiveness as it relates to specifc student needs. Offer and implement changes when warranted.
- Collaborate with teacher to address student achievement de?ciencies focusing resources, materials, and technology to promote satisfactory classroom performance.
- Assist in monitoring and adjusting the instructional process to ensure program activities are aligned with desired outcomes
- Schedules, or assists in scheduling, 504 meetings and notifes parents of the 504 meeting
- Distribute 504 calendar to appropriate 504 Committee members in a timely fashion
- Ensures all appropriate elements are completed in applicable cases
- Notify all appropriate school personnel of 504 meeting/schedule changes
- Ensure 504 plans are being followed as mandated
- Coordinate school, home, and community resources and refer students, parent, and others to special programs and services as needed
- Work collaboratively to advocate for individual students and specific groups
- Establish and maintain a positive relationship with principals, teachers, staff, and parents.
- Articulate the district’s mission and goals to all stakeholders in the interest of student success and solicit support in realizing the mission.
- Compile, maintain, and file all reports, records, and other documents as required.

Primary Purpose: Manage a caseload of students with regard to programming needs and collaborative efforts between service providers, administration, instructional personnel, and parents/guardians to provide supportive services and supports based on assessed needs. The facilitator is responsible for coordination, planning and preparation for compliance documents for students eligible for 504 and Special Education Services.Qualifications:
Education/Certification:
Bachelor’s degree (preferred) Master's degree (preferred)
Special Knowledge/Skills/Abilities:
Knowledge of state and federal laws for educating students with disabilities
Understanding of the individual needs of students with disabilities.
Ability to communicate effectively with parents and staff.
Ability to implement policies and procedures
Ability to interpret data
Strong organizational, communication, and interpersonal skills
Experience:
With 504/SPED policies and procedures and/or providing social emotional support to students in an agency or educational setting
