Overview

As a Social Services Coordinator you will work with the Nurse Coach team helping patients find resources and items they need upon discharge.

CareCentrix's extensive range of services provides support and coordination for patients and their families in every step of the healing process, including home health, durable medical equipment (DME), home infusion, sleep management, and care management services, which ultimately improve care transitions and reduce unnecessary hospital readmissions, and emergency room visits.

Responsibilities

In this Job, you will:

• Obtain and share information on benefits and community resources to support patients after discharge from acute and post-acute facilities.
• Assist in sending Medicaid applications, reduced-cost drug applications, and any additional documentation or forms that are required to help patients access and receive the benefits to which they are entitled through their Health Plan.
• Collaborate with CareCentrix nurse coaches to support patients in the home and ensure patients have information on community resources that are available to support their recovery such as: meal support, transportation to primary care physician appointments, financial assistance with medical bills and access to their medications, etc.
• Engage with the Health Plan to obtain the necessary historical or eligibility information to help patients access their appropriate benefits and resources
• Provide training and support to the Readmission Management team on available Health Plan benefits and community resources.
• Participate in CareCentrix team meetings, as needed.
• Adhere to the mission, vision and values of CareCentrix.

This is the job for you if:

• You have strong technical skills with proficiency in Microsoft Office, previous experience with electronic medical records (EMR), and ability to navigate using multiple screens and systems.

Qualifications

• You hold a degree in social work or other related field
• You have a deep knowledge of community amenities and resources
• You hold a deep knowledge of planning and outstanding knowledge of medical terms
• You have experience and the ability to recognize the Health Plan benefits and the community resource needs of the patients and caregivers.
• You possess 5+ years’ experience in community resource management or Health Plan benefit management.
