Summary: The Transfer Coordinator answers the phone, accurately records & delivers all messages to & for the Case Management Employees & Managers. Assists the Case Management employees with the coordination of hospital patient transfers to appropriate post-acute facilities which includes ordering equipment and making referrals. Assists the Social Workers with clerical job functions. Performs all clerical job functions, i.e. filing, faxing, maintaining office supplies, computer trouble-shooting, committee minutes and agendas, creating data bases, compiling resource information, booking meetings, mail, etc. Meets with all vendors (Durable Medical Equipment (DME)/Respiratory/IV companies to establish relationships and streamline referrals. Interacts with all post-acute facility/agency liaisons to improve collaboration for transitions across the continuum of care and secures bed placement. Completes assignments and schedules under the direction of the Manager. Interacts with all third party payers to prevent denial of payments under the direction of the Case Manager. Distributes Important Messages to patients. Notifies other departments regarding discharges, transfers, admissions, observations etc. Reviews EMR/Medical Record for information as requested.
Education: Minimum High School Diploma. A college degree in Business/Health Administration or equivalent diploma is preferred.
Experience: Minimum of five (5) years Healthcare/Hospital experience. Computer skills and interactions with patients and medical personnel in a healthcare setting. Excellent communication skills required.
