Carry a primary caseload providing CTI services.

- Bachelor’s degree preferred, Associates required.
- One year employment experience in the Human Services field.
- An equivalent combination of training and experience may be substituted for the educational and employment requirements.
- Experience and/or training in the care coordination for those with severe mental illness and substance use disorders preferred.
- Basic computer skills required.
- Must have a valid NYS driver’s license.

Overview
To support the Mission of Helio Health, Inc., the CTI Case Manager will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI is designed specifically to enhance the continuity of care during transition from hospital to community care.
CTI Case Manager:
Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Case Manager is a care manager, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual’s ability to meet recovery goals. The case manager will work on a Full-Time basis and using a hybrid work model, working in the community and out of Helio Health’s Rochester Outpatient Program.
The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities.
Pay Rate: $23.00-$24.52 per hour.
In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!
