- Assist clients with initial screening, intake, authorization, admission, successful utilization of services, review of services, and planning for graduation.
- Review Initial Referral Forms for completion, accuracy and match to client’s stated interests
- Provide Services consistent with Trauma Informed principles of safety, trustworthiness, choice, collaboration and empowerment.
- Provide at least three, up to six groups a day with evidence-based curricula that is also responsive to the emerging and dynamic needs of the group members
- Participate in analysis of data-driven and qualitative feedback with the goal of improving service and outcomes
- Document required information in the Electronic Health record on the same day as service
- Collaborate with team in the recruitment of new and retention of existing Day Program Clients
- Provide de-escalation services as needed
- Plan and evaluate services with the client and their care team in a spirit of collaboration and with a focus on the client’s stated recovery goals.
- Models and reinforces Community Connections mission to provide behavioral health, residential services, and primary health care coordination for marginalized and disenfranchised women, men, youth, and children living in the District of Columbia, many of whom are coping with challenges including mental illness, addiction, and the aftermath of trauma and abuse.

- Protects the privacy of our consumer’s protected health information by maintaining compliance with HIPAA and other relevant CC related IT security regulations.
- Completes and stays current on role specific and organizational wide training.
- Performs other duties as assigned on an as-needed basis.
- Bachelor’s degree in social work, psychology, or related industry required
- Experience in mental health setting preferred.
- Experience facilitating groups preferred
- Familiarity with DC community resources preferred
- Knowledge of Microsoft Office to include Outlook, Word, and Excel.
- Ability to effectively communicate and present information to clients, consumers, staff, and the public.
- Ability to recognize and maintain professional and personal boundaries with consumers and peers.
- Ability to read, analyze, and interpret documents
- Excellent organizational skills and attention to detail; ability to prioritize multiple tasks and meet frequent deadlines
- Excellent oral and written communication skills.
SALARY RANGE: 40,000-55,000 (Salary commensurate with experience)
ONSITE POSITION
POSITION SUMMARY
The Day Rehabilitation Services Specialist provides group and individual intervention to the cohort of clients in the Day Services Program. Assistance is provided from referral to discharge by completing screening and intake, authorization documents, individual interventions, and group-based services that are designed to support an individual’s recovery. Group services are based on Evidence Based Curricula. The Day Rehabilitation Services Specialist works in collaboration with other Day Rehabilitation Service Specialists, the Program Director, referring parties, and the client’s care team. The Specialist provides services in a Culturally Responsive, Trauma- Informed manner, consistent with Evidence Based Practice. All interventions with clients and other staff are delivered in the spirit of respect and support for recovery.
