      Thriving non-profit, federally qualified health center headquartered in Palatka, Florida is seeking a motivated and experienced Dental Hygienist.

      This is a full-time position. Monday-Thursday,8:00 am-7:00 pm

      POSITION SUMMARY

      Provide the full scope of primary care oral health services which fall under his/her field

      of training, including, but not limited to, preventive services and oral health education to

      patients of Aza Health (AH). In addition, the Dental Hygienist is a member of the AH

      team. In that capacity, he/she is expected to be familiar with the philosophy, goals and

      objectives of AH and work collaboratively to achieve them. He/she must be cognizant

      of, and comply with, all AH policies and procedures, as well as pertinent state and federal

      regulations. This position reports to the Chief Dental Officer.


      POSTION RESPONSIBILITIES

      • Prepare treatment room for patient by adhering to prescribed procedures and

      protocols (i.e. - Operatory Protocols for Hygiene Visits, Operatory Cleaning

      policy, Instrument Sterilization policy, etc.).

      • Complete patient screening procedures as appropriate (i.e. - assessment of oral

      health conditions, review of health history, oral cancer screening, head and

      neck inspection, taking blood pressure and pulse, etc.)

      • Complete dental prophylaxis by removing calculus and plaque (hard and soft deposits) and stains from all surfaces of the teeth and from beneath gum

      margins.

      • Teach patients appropriate oral hygiene strategies to maintain oral health

      (e.g., tooth brushing and flossing).

      • Counsel patients about good nutrition and its impact on oral health
      • Arrive and be available to see patients on time. This applies to morning and

      afternoon sessions. The goal is to minimize patient waiting time and increase

      patient satisfaction.

      • Complete clinical record documentation in the electronic health record (EHR)

      prior to leaving the office on the day the service is performed, with charts

      locked within 72 hours of visit.

      • Comply with all AAAHC (Accreditation Association for Ambulatory Health

      Care) requirements (medication reconciliation, avoid “Do Not Use”

      abbreviations, pain management, time-out, etc.).

      • Comply with all AH credentials requirements (e.g. - PPD testing on initial

      hire, DEA, FL licensure, 3rd Party insurance applications, etc.).

      EDUCATION

      • Graduation from an accredited Dental Hygienist Program.


      CERTIFICATES AND LICENSES

      • State of Florida licensure
      • Basic Life Support Certification

      SKILLS

      1. Language – Bilingual (English/Spanish) preferred, but not required.

      2. Computer literate - ability to utilize electronic health record in the

      performance of clinical duties

      3. Ability to analyze complex information and communicate effectively both

      orally and in writing.

      4. Excellent interpersonal skills to establish effective and harmonious relations

      with AH patients, AH staff and the general public.

      5. Commitment to and ability to successfully demonstrate honesty,

      confidentiality, integrity, quality, teamwork, customer service, and

      professionalism. Must have and display a positive outlook. Must demonstrate

      dedication to the Aza Health mission and to the success of the corporation.
