Job summary
We are looking for a Diabetes Specialist Nurse with skills, to provide expert nursing care and clinical leadership in the management of patients with diabetes. The post holder will work autonomously and as part of the multidisciplinary team to deliver high-quality, evidence-based care, ensuring patients receive personalised treatment and education to support safe, independent diabetes management.
Main duties of the job
The post holder will deliver efficient, professional, and equitable diabetes care to our practice population. They will assess patient'sneeds, offer expert advice on complex diabetes management, and support the clinical team. A key responsibility is developing, implementing, and reviewing personalised care plans, including initiating and adjusting insulin doses.
They will monitor medication effectiveness, ensuring treatments are safe, evidence-based, and meet individual needs. The role involves providing practical and emotional support to patients, particularly those newly diagnosed or experiencing complications.
The Diabetes Specialist Nurse will work closely with patients to support lifestyle changes and encourage adherence to treatment.
Strong communication is essential, as the role includes discussing sensitive issues with patients, families, and professionals. Home visits may be required for reviews or added support. Advanced clinical skills will be used to assess, diagnose, and manage care, with ongoing adjustment to meet changing needs.
Accurate record-keeping is vital, alongside involving patients and carers in planning care. The post holder will work closely with GPs, nurses, HCAs, pharmacists, and external partners to ensure continuity. They will also act as a clinical resource, support staff development, provide patient education, and promote self-management, prevention, and early intervention.
