      Job summary

      The post holder will be required to provide a comprehensive and integrated specialist nursing service

      for people with diabetes both in the Hospital and in the Community.

      The role aims to drive parity of esteem strengthening the diabetes service offer and partnership

      approaches to ensure equal access, outcomes and experience in diabetes.

      The post holder will join other colleagues providing a service for adult people with diabetes they will

      act as an additional resource for nursing colleagues and other health workers offering expert care,

      advice, teaching and support and building up communication links as needed. Flexibility is essential in

      this post.

      Main duties of the job

      Main duties include providing an inpatient diabetes service. Assessing and reviewing acutely unwell

      patients to enable they have a positive patient journey.

      Ensuring that they are receiving high quality diabetic care as well as ensuring that their medications are

      appropriate and patient centred to achieve good outcomes and reduce long term effects of diabetes.

      Supportingpatients with the available technology eg CGM, Flash glucose monitoring and insulin pumps.

      Empowering patients to take control of their diabetes and promote self management. Influencing

      practice and partnership working across primary care, secondary and mental health and learning

      disability services

      We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

      Job description

      into the most efficient use of their diabetic equipment.

      2. To develop the awareness of people with diabetes and relatives/carers about factors

      contributing to the promotion of good health/improvement in the management of diabetes, to ensure

      involving pre and post discharge visits.
