      Job summary

      The Norfolk Community Health and Care NHS Trust community diabetes team is based in West Norfolk, we are a small friendly team who provide care for people living with diabetes in a variety of settings, in clinics in patients homes and in care settings.

      You will work within the Diabetes Team providing expert knowledge and skills.

      Main duties of the job

      To work within the Diabetes Team providing expert knowledge and skills to:

      • Deliver a specialist Diabetes Service to community care, clinics, nursing/care homes and housebound patients across the primary/secondary interface which compliments and enhances the existing community based service.
      • Promote and raise the profile of diabetes management within local trusts, to professionals, patients and their carers.
      • Support the improvement of standards of care for people with Diabetes in line with the National Service Framework (NSF) for Diabetes.

      The community diabetes service is a busy nurse led team, we work across west Norfolk in assessing and managing people with complex diabetes treatments.

      We offer support and education to other health care professionals and run structured education sessions for patients. We offer the opportunity to work flexibly within the service, and have opportunities to develop and learn new skills.

      To deliver a specialist care and support service within West Norfolk including:

      Provision of education and support to patients who have diabetes in order to enable them to self-manage and reduce their risk of complications from diabetes.
