      Job summary

      We are seeking a dedicated and knowledgeable Diabetes Nurse Specialist to join our healthcare team. The ideal candidate will have a strong background in patient care enabling them to provide exceptional support and education to patients managing diabetes.

      This role is crucial in helping patients understand their condition, develop effective management strategies, and improve their overall health outcomes.

      You will be working closely with a friendly supportive team of Doctors, Practice Nurses, HCAs, Urgent Care Practitioners and Care Coordinators, delivering service to over 23000 patients across our three sites in the north of Bristol.

      Job description

      Job responsibilities

      • Play an active role in the design and implementation of a holistic diabetes service.
      • Put the patient at the center of the diabetes service.
      • Take lead clinical responsibility for crafting treatment plans.
      • Review diabetic blood test results.
      • Provide treatment plans based on current best evidence.
      • Screen incoming letters from secondary care and action recommendations
      • Encourage patients to take ownership of their condition.
      • Co-ordinate the provision of care to nursing home patients.
      • Co-ordinate the provision of care to house bound patients.
      • Provide a supporting role to the research lead GP for research relating to diabetic care.
      • Provide clinical supervision for clinicians working in the diabetes team (Nurses, HCAs etc).
      • Work to maximise referrals to the NDPP.
      • Work to maximise achievement of QoF for Diabetes

      Qualifications

      Essential

      • Registered General Nurse
      • Professional Knowledge acquired through degree supplemented by diploma level specialist training.
      • Nurse independent prescriber.
      • Experience of insulin starts in Type 2 patients and worked with Type 1 patients, assisting to titrate insulin.

      Desirable

      • Experience working with EMIS and Ardens templates.
      • Knowledge of/worked with freestyle libre 2.
      • Experience of running diabetes clinics in a community or primary care setting.
