What to Expect (Job Responsibilities):
- Provide remote digital in-basket management, including triage, lab review, and prescription refill responses
- Conduct limited telehealth visits in collaboration with an on-site clinical support team
- Review and manage patient information including labs, diagnostic images, and other medical records
- Communicate effectively with on-site staff for patient care coordination and scheduling
- Document all relevant patient information in the electronic health record system

What is Required (Qualifications):
- Minimum of 3 years of experience for NP or PA-C candidates; 1 year for MD or DO candidates
- Graduate of an accredited Master of Science in Nursing program or Physician Assistant Program, or MD/DO licensed in California
- Current Family Nurse Practitioner certification for FNP applicants and current Physician Assistant License for PA applicants
- Must have or obtain current California medication furnishing license and DEA License
- Strong verbal and written communication skills with excellent interpersonal abilities

- Experience in a community healthcare center
- Bilingual (English/Spanish) language skills
- Ability to treat patients with complex medical and social needs
- Strong problem-solving skills
- Familiarity with Electronic Health Records, specifically EPIC
