      Provides leadership for Holy Cross Hospital in developing, strengthening and enhancing multicultural education, relationships and partnerships with community agencies, grassroots organizations, schools, churches, elected officials, healthcare providers, and other key stakeholders in the hospital’s service areas.

      HCH Director, Strategic Hospital Partnerships supports leadership in the development and implementation of initiatives aimed at reducing health disparities, with a strong focus on cultural competencies, creating an inclusive and collaborative environment with partners, patients, families and caregivers. (Emphasis on outreach to community multi-cultural populations)

      The HCH Director, Strategic Hospital Partnerships oversees (coordinates) the hospital’s participation in community and institutional collaboration. Organizes events and educational sessions to prevent and treat chronic disease, engages community members and partners in the System’s programs and services, and builds market share of minority populations to make HCH their first choice of care. This position reports to the Vice President of Mission.

      Key Job Activities:

      · Develops and implements strategies to reach out and collaborate with community stakeholders to address healthy lifestyles, access to health care, health inequities and community wellness in HCH service areas.

      · Serves as a liaison between community partners and the HCH departments, resources, programs and services to coordinate and oversee HCH’s participations and collaborations through health fairs, educational sessions to prevent disease, and other as needed and as requested by partners.

      · Provides internal feedback to the hospital in regards to cultural competence, appropriate linguistic services, and welcoming environments for patients and clients, with an emphasis on populations of immigrants and non-English speakers.

      · Disseminates the SHS mission, vision and values, and increases HCH’s visibility and market share, especially among minorities.

      · Represents HCH at relevant community initiatives, councils, boards, roundtables, associations and consortiums.

      · Assists HCH in conducting the System´s response to special projects or events affecting the communities in services areas (i.e. Covid-19, Census, immigration policies, and other.

      · Coordinates and manages HCH collaboration with service area institutions, churches, schools and businesses.

      · Collaborates with and participates as a member of the SUHI Community Advisory Council (CAC) to engage and improve community services and programs.

      · Collaborates with other Sinai/HCH departments in seeking and submitting proposals to request private and public grants to finance programs and initiatives as convenient for the Department and other System´s initiatives.

      · Participates in internal task forces and teams that request the input of Community Relations to enhance programs, develop new ones, hire personnel, conduct marketing and public relations activities, celebrate cultural festivities, and other.

      · Creates outlets of communication to inform Sinai (/HCH) partners, community members, and staff, in English and Spanish, of programs, services and events (i.e. email blasts, social media, video calls.)

      · Represents the System, as requested by Marketing/Public Relations, in media interviews or other communications in Spanish, as a bilingual-bicultural leader.

      · Performs other duties as assigned.

      Education and Work Experience:

      · Bachelor’s Degree required. Master’s degree preferred.

      · Minimum five years of related experience in community outreach/education. Experience in prospect research, grant development and writing, preferred.

      Knowledge and Skills:

      · Ability to connect with others and forge strong relationships.

      · Ability to perform several tasks concurrently, time management and organizational skis.

      · Ability to maintain confidential information.

      · Excellent communications skills required.

      · Ability to organize and maintain detailed records, complete necessary paperwork and meet deadlines.

      · Experience Microsoft Office programs and tracking database.

      · Spanish speaking a plus.

      · Experience in EPIC and MEDITECH software systems preferred.

      Category 1 - Job usually involves contact with patients or patient’s specimens. Exposure to blood, body fluids, non-intact skin or tissue specimens, is possible

      Category 2 - Job may expose incumbent occasionally or in emergency situations to blood, body fluids, non-intact skin or tissue specimens

      Category 3 - Job does not involve exposure to blood, body fluids, non-intact skin or tissue specimens. Incumbent does not perform or help in emergency medical care or first aid as a part of his/her job.

      Registry (per diem) Caregivers are employees who work less than 1,000 hours in a calendar year but are typically on an as needed basis. These caregivers may be called upon to cover full-time or part-time caregivers during PTO events or sick leave as well as to provide supplemental coverage. Registry Caregivers are not eligible for Sinai’s benefit programs with the exception for City of Chicago Paid Leave and Paid Sick Leave (Sick-C), PPO Medical Plan, Employee Assistance Program (EAP) and non-match eligible contribution to the Sinai Health System Consolidated 403(b) retirement savings plan.
