- Maintain the health and well-being of the individuals Empowering People’s Independence (EPI) serves, to include administration of medications as directed per care plan.
- Implement and adhere to behavioral support and treatment plans, and personal goals as directed in care plan.
- Maintain a clean and safe living environment for persons supported through cleaning and physical upkeep of residence.
- Help persons supported maintain their household by assisting with laundry, budgeting, and shopping.
- Assist with daily living activities, including preparing meals, assisting with personal hygiene, participate in outings, and toileting as needed, self-care, social, recreation, community, domestic skill to help reach goals as outlined in their care plan.
- Work shift schedule as assigned to ensure the proper staffing levels.
- Provide positive role modeling with individuals and co-workers.
- Transport individuals when needed to medical appointments and recreational activities in accordance with EPI organizational and departmental policy.
- Maintain accurate, up-dated charting and documentation per EPI policy and governmental regulations.
- Report all policy violations to appropriate personnel.
- Attend staff meetings as directed to evaluate and promote effectiveness of both the individual program and the general program.
- Promote workplace safety at the residential homes and while transporting individuals.
- Performs other duties as assigned by Supervisor and/or designee within scope of practice
- Participate on various committees as able.
- Volunteer on various EPI Marketing or community events and/or Epilepsy Camps that are not in conflict with EPI.
- Maintain the following required certifications & learnings within 90 days of hire and annually thereafter;
- CPR-First Aid
- SCIP-R ( Strategies for Crisis intervention & Prevention Revised)
- AMAP (A pproved Medication Assistive Personnel).
- OPWDD, REGULATORY & EPI learnings
- Maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors’ names, investigations, finance, operations, and employee relations matters.

- High school graduate or equivalent; one-year experience in the human service field preferred.
- Strong working knowledge of Microsoft programs to include Email and Microsoft Teams.
- Ability to navigate electronic documentation program (i.e. Medisked, Evero).
- Ability to use online systems to input work hours using electronic timekeeping system and complete online training.
- Maintain a valid New York State driver’s license. You will be required to drive company vans to transport individuals and consistently pass vehicle safety training.
- Must be able to demonstrate effective verbal and written communications skills.
- Must be able to work collaboratively with co-workers and clinical team.

- Must be able to talk, hear, stand, crawl, walk, and/or sit for extended periods of time; may have to twist, bend, use hand to finger, stretch with hands and arms.
- Specific vision abilities required by this job include close vision and ability to adjust focus.
- May be exposed to strong odors from time to time.
- Must be able to lift and carry up to 50 lbs.

Pay Rate - $19.50/hr-$25.00/hr
