      Job Overview:

      We are seeking a highly motivated Director of Community Health Worker Management to oversee our team of community health workers who will work to address social determinants of health (SDOH) needs and connect patients with physicians for clinical care.       Qualifications:

      Required:

      • Bachelor's degree in healthcare management, public health, or a related field.

      • Minimum of 5 years of experience in managing a team of healthcare professionals, preferably in a community health setting.

      • Excellent leadership, communication, and interpersonal skills.

      • Strong understanding of healthcare policy and regulations.

      • Ability to work collaboratively with physicians, healthcare professionals, and community-based organizations.

      • Knowledge of various social determinants of health modeling and experience working to address them in a community setting.

      • Knowledge of using Electronic Medical Record; i.e. EPIC/Cerner/Allscript.

      • Experience in developing and implementing community health programs.

      Preferred:

      • Master's degree preferred
      • Bilingual (English/Spanish) preferred

      Essential Job Functions:

      • Manage and oversee a team of community health workers who will be responsible for identifying and addressing SDOH needs in the community, as well as connecting patients with physicians for clinical care.
      • Develop and implement effective strategies to ensure that the team is meeting performance goals and providing high-quality care to patients.
      • Establish and maintain partnerships with local community-based organizations to enhance the delivery of healthcare services to patients.
      • Collaborate with physicians and other healthcare professionals to ensure that patients receive comprehensive care.
      • Provide ongoing training and support to community health workers to ensure that they have the skills and knowledge needed to provide effective care to patients.
      • Develop and implement policies and procedures to ensure that the team operates efficiently and effectively.
      • Participate in the development and implementation of community health programs, and collaborate with other healthcare professionals to ensure that these programs are meeting the needs of the community.
      • Stay up-to-date with developments in healthcare policy and regulations, and ensure that the team is complying with all relevant laws and regulations.
      • Develop strategy to deploy Community Health Workers in in-patient and ER settings in the local RWJBH Hospitals and partner with various stakeholders to impact the avoidable readmission.
      • Develop Key Performance Indicators to measure the success of the program and able to provide reports to senior leadership of the team’s success periodically.
