- Conducts initial assessment of prospective residents, including off-site visits
- Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident
- Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents
- Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
- Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members
- Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences
- Coordinates the Community’s infection control program while maintaining state regulatory compliance
- Develop and oversee the Community’s medication management program
- Contributes toward quality assurance and performance improvement initiatives
- Complies with all rules and regulations regarding confidentiality and privacy of resident information
- Maintains current state nursing licensure. LPN required, RN preferred
- Minimum of two (2) years nursing supervisory experience
- Prior senior/supportive living experience strongly preferred
- Excellent written and verbal communication skills
- Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members
- Demonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignity
