      QCH’s Older Adult Services/Older Adult Centers offer multi-faceted programming (5 senior centers, case management and HDML, NORC, Social Adult Day Care and Caregiver Support and Transportation Services) allowing seniors to age in place with dignity, while encouraging independence and engagement with the community. Our programs recognize that the older adult community represents a wide spectrum of the aging process, a spectrum of needs, strengths, and interests.

      Social Adult Day Services programs offer support, social engagement, and therapeutic activities to older adults who are physically frail, socially isolated and/or memory-impaired. These programs are ideal for individuals who live at home, but need a little bit of extra support and/or companionship during the day. They also provide respite to family members and other caregivers.

      QCH has more than 30 years of experience providing high-quality social daycare services in a safe, warm, and welcoming environment. With the support of trained and caring staff, participants engage in supervised educational and recreational activities that increase their confidence and feelings of self-worth.

      The program director will oversee two distinct program models:

      • LIFT (Living Independently with Friendship and Togetherness) is for individuals who are physically and/or emotionally frail and/or exhibiting mild memory impairments. LIFT meets Monday through Friday from 9:30 a.m.-2:30 p.m.
      • TYME AWAY is for individuals who are living with moderate to early late-stage Alzheimer’s disease or related degenerative dementia disorder. TYME AWAY meets Tuesdays and Thursdays, from 9:30 a.m.-2:30 p.m.

      Responsibilities

      • Develop and maintain engagement activities suited to meet the emotional, physical, intellectual and social needs of participant and caregiver individuals/groups.
      • Determine participant appropriateness for programs.
      • Oversee and perform intake, assessments, reassessment as well as home visits and their review.
      • Coordinate with administration to ensure compliance with Building, Fire, and DOH codes.
      • Facilitate family and/or caregiver participation by providing/identifying support services as neded.
      • Oversee participant care plan delivery.
      • Facilitate Caregiver Support Group.
      • Support program staff, as necessary, in providing direct assistance to participants with meeting their ADL needs, escorting them to/from program areas, bathroom, and transportation services as needed.
      • Supervise program staff, volunteers, and interns.
      • Cooperatively develop of program staff job descriptions.
      • Manage evaluation process for program staff/interns.
      • Develop training plan and track staff training hours for SADS.
      • Plan and lead regular staff meetings.
      • Develop and maintain, with OAS AED and Deputy Director, SADS funding.
      • Adhere to NYSOFA and NYC DFTA standards including HIPAA standards.
      • Cooperatively review program and staff performance/needs.
      • Oversee the tracking of SAD program outcomes
      • Cooperatively work with staff and upper management to increase program visibility and outreach efforts.
      • Identify opportunities to improve the tracking methods of program outcomes.
      • Contract and Budget Performance
      • Supervise the completion of monthly program statistics, participant invoices, reimbursement forms, and other reports and fiscal tasks as necessary.
      • Oversee and assist staff with identifying planned expenditure opportunities.
      • Oversee and assist staff in identifying and planning for expenditures, including program supplies, equipment, and subcontracted services.
      • Actively participate in professional development plan.
      • Attend monthly Older Adult Center Directors’ and partners’ meetings, other agency, DFTA, NYSADSA, and/or NYSOFA meetings as appropriate.
      • Serve on appropriate internal committees as they relate to the programs.
      • Identify training and conference interests for incorporation into annual development plan.
      • Report directly to Deputy Director of Older Adult Services.
      • Supervise the coordination and management of virtual schedules and activities, as well as related virtual links to program members and their families.

      Qualifications

      • Masters in Social Work or a related field preferred, licensure is preferred.
      • Minimum of two (2) years of experience working with older adults in an adult day care program or similar setting.
      • Superior written and verbal communication skills.
      • Excellent interpersonal skills with demonstrated ability to build and sustain relationships.
      • Self-motivated and resourceful; able to work independently as well as collaboratively with staff.
      • Ability to maintain confidentiality and use appropriate discretion.
      • Proficient in Microsoft Office applications including Word, Excel and Outlook and database management/search tools. PeerPlace and Simple Servings web-based programs knowledge is a plus.

      Salary : $60,000 - $65,000
      Schedule: 35 hours per week
