      Job Title: Director of Social Services
      Location: Bedrock Rehabilitation and Nursing Center at Wedgewood, Lakeland, Florida
      Shift: Full Time
      Salary: $60,320.00 annually

      About the Role:

      Bedrock Rehabilitation and Nursing Center at Wedgewood is seeking a compassionate and experienced Director of Social Services to lead and oversee the social service programs of our facility. You will play a vital role in enhancing the quality of life for our residents by planning, developing, implementing, and evaluating social service initiatives.

      Key Responsibilities:

      • Interview residents and family members to obtain social history and assess needs.

      • Provide guidance on Medicare, Medicaid, and other financial assistance programs.

      • Refer residents/families to appropriate external social service agencies as needed.

      • Collaborate with staff and departments to standardize work processes, improve workflow efficiency, and reduce costs.

      • Develop and update departmental policies, procedures, and job descriptions.

      • Coordinate transportation arrangements for residents when necessary.

      • Document detailed social service progress notes and track resident responses.

      • Attend Quality Assurance and Performance Improvement (QAPI) meetings and participate in corrective action planning.

      • Maintain a comprehensive community resource referral file.

      • Interpret and communicate departmental policies to residents, families, employees, and government agencies.

      • Stay current with federal/state regulations and recommend policy updates to the Administrator.

      Qualifications:

      • Proficient in reading, writing, speaking, and understanding English.

      • Strong decision-making skills with the ability to act independently when needed.

      • Knowledge of and ability to provide age-appropriate care and support to residents.

      • Excellent teamwork and leadership skills; able to supervise and collaborate with staff effectively.

      • Exceptional interpersonal skills; able to tactfully interact with personnel, residents, families, visitors, and government officials.

      • Patient, tactful, cheerful, and enthusiastic approach to resident care, regardless of cognitive ability.
